Why Do We Get “Off-Balance”?

We maintain our balance because of an integration of complex systems in our body that perceive, transmit, and process information about the body’s position in relation to the ground, to space, and to what is considered normal alignment. All of this causes certain muscles to work and reflexes to fire, “keeping us upright”.


There are a variety of reasons why balance may become impaired:

  • Vestibular Disorders including: labyrinthitis, neuritis, inner ear post-surgical cases, and Meniere’s disease
  • Positional Vertigo—BPPV
  • Neurological Disorders including: stroke, Cerebellar dysfunction, MS, and Parkinson's disease
  • General weakness and decreased range of motion of legs and trunk
  • Lower extremity injury or surgery
  • Dysequilibrium
  • Postural Instability
  • Visual Deficits
  • Peripheral Neuropathy


Treatment Approach

After a detailed evaluation, the treatment plan is tailored to meet the patients’ individual needs and goals. Most patients will attend therapy 1-2 times per week. Clinic visits focus on advancing the patient’s home exercise program, and increasing the complexity of the program. For the majority of conditions treated the program lasts about 8-10 weeks, though some may be resolved in 1-2 visits.


Components of the program can include:

  • Adaptation Exercises
  • Habituation Exercises
  • Substitution Activities
  • Proprioceptive Neuromuscular Re-education
  • Balance Training Activities
  • Gait Training
  • BPPV Positioning Treatment
  • Therapeutic Ball Activities
  • Trunk and Lower Extremity Strengthening Exercises


Will this Program Really Help My Balance?

Balance can improve in most cases. Challenges to the entire balance system (visual, vestibular, and sensory) are important for developing new strategies of movement and postural control, as well as retraining old ones. This program is essential for those with recent falls or who experience progressive problems with balance.


The dizziness, or vertigo, produced in vestibular disorders can also be treated. Vestibular problems that are acute or those that have been chronic for 6 months or more do respond to this type of treatment.
